Residential Treatment
Overview:
Transitions, Inc. believes that addiction is a disease, and treats Substance Use Disorders from the disease model. Treatment is individualized to best meet the needs of the consumer. Treatment is provided through trauma-informed, person-centered and recovery-focused services.
Our Residential and Outpatient Programs follow evidence-based practices. These practices indicate that providing individualized treatment is most successful when intervening with individuals with a Substance Use Disorder. What this means is that each person's treatment plan may look a little different, which directly relates to the length of stay in treatment at various levels of care.
We start our treatment plans by identifying the appropriate level of care for our consumers based on ASAM (American Society of Addiction Medicine) Criteria. Facilitating a thorough bio-psycho-social assessment, we identify a client's needs, strengths, psycho-social history and problem areas. We then begin to develop a treatment plan. Once referred to the correct level of care and treatment program, we continue the planning process by identifying what it takes to assist our clients with a successful reentry into the community, living a safe and sober life. Length of stay and successful completion of treatment depends on each individual's needs. It depends on many psycho-social factors, but ultimately it is the progress made by the consumer that determines a projected discharge date. Discharge dates are not set in stone. Dates can change based on a consumer's progress throughout the treatment program. When evaluating a consumer for projected discharge, Transitions clinicians work in teams to review consumer files to make sure the consumer is working toward, and successfully completing the goals set in the initial treatment plan.
